Description: Under the general supervision of the Vice President of Patient Care/Chief Nursing Officer and the direct supervision of the Community Case Management Coordinator, the Case Manager performs the following functions across the continuum of care: assessment, planning, implementation, coordination, monitoring and evaluation The case manager functions as a patient advocate, uses problem solving techniques and collaborative referrals to facilitate accessing Network and community resources The Case Manager will provide case management services to a defined population of clients Work requires the knowledge and skills necessary to provide case management to varied age groups
