Responsible for the utilization of high quality, cost-effective services provided to a defined group of patients. 1. Reviews and approves the patient’s plan of care for all new admissions based upon standardized guidelines and the individual patient need. Baccalaureate of Science in Nursing preferred. Graduate of NLN approved program or an accredited program in a health care related field or equivalent experience. Must be a licensed clinician, if required, in Vermont and New Hampshire. Minimum of 3-5 years of home care experience preferred.
