Responsible for the delivery of professional occupational therapy services in the patient’s home, which will address the physical, psycho-social, spiritual, cultural, developmental, and age-appropriate needs of each individual patient/family. Assists patients in achieving optimum levels of functioning in self-care, work, school, home-making and/or leisure activities. Under the direction of the physician, the occupational therapist plans a therapeutic program to develop the patient’s motor, cognitive, sensory-integrative, psychological, and social functioning needs. Occupational therapist will utilize physical means of sound, light, electricity, water, heat, cold, exercise, splinting, and manual interventions. Therapist will be responsible for documenting care plan, rehabilitative potential, and discharge planning needs. Evaluates and provides treatment, implements and coordinates care with team members.
