The Physical Therapist evaluates, develops, implements and documents an intervention plan; provides intervention, evaluating and documenting progress toward goal; develops and documents a discharge plan; provides supervision for therapy assistants and other aides; provides consultation and education for patient, families, referral sources, and community agencies; participates in growth and development of service area, new programs, review of existing programs and maintains positive lines of communication with contract facilities continually promoting our team of therapy services. Patients cared for include infants, children, adolescents, adults and geriatric.
