Physical Therapy Asst

Graduate from school of Physical Therapist Assistants required. Recent experience (minimum two years) in acute care and/or rehabilitation center required. Healthcare Provider CPR required at date of hire. Shift Hours: 8AM-5PM.The Physical Therapy Assistant (PTA) is responsible for providing physical therapy services to patients in the home under the direct or indirect supervision of a physical therapist, in conjunction with the patient, family and interdisciplinary team, following the plan of treatment as ordered by the physician.  The PTA will be responsible for clinical outcomes as well as industry benchmarks. Supervision must be performed by a registered physical therapist every five (5) visits, or once a week if treatment is performed more than once a day. Regularly monitors patient status. Addresses all problems identified in the Plan Of Care (POC). Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition. Communicates with other disciplines involved in the case as warranted. Reports signs/symptoms signaling a change in patient condition to the PT, Case manager and physician as needed. Responsible for continuous review of all aspects of every patient on his/her caseload including: a.Appropriate utilization of services; b.Ensuring continued skilled need; c.Monitoring homebound status; d.Reviewing documentation in the medical record; e.In conjunction with PT,  provides updates to the physician regarding the POC, including orders; f.In conjunction with PT,  provides updates to the insurance case manager, including obtaining insurance authorization when necessary; g.Reporting of low utilization patients, significant changes in condition and partial episodes, patients with infections, wounds and fall risks as appropriate; h.Discharge planning in conjunction with PT. i.OASIS outcomes; and j.Other industry benchmarks. Accountable for daily assignment, notifies supervisor immediately if patient assignment changes.  Proactive in managing schedules of patients on assigned case load and coordinating 5th visits with PT.  Coordinates schedules the previous day with the scheduling personnel.  Is flexible with scheduling to allow PTs to accommodate new admissions into zip code territory.
