RN Care Management Coordinator
Location: Missoula - Montana
Rate: N/A (Permanent) + Healthcare Insurance Benefits Equivalent to N/A + Healthcare Insurance Benefits
Works with all appropriate members of the healthcare team to ensure the coordination of patient care through the continuum, working to achieve specific patient outcomes and track, analyze and trend variance as they occur. The Case Manager coordinates communication between all caregivers to identify patient needs early and intervenes in a timely and appropriate manner. This will include patient and family assessment, monitoring, utilization management and discharge planning Current Registered Nurse Licensure in Montana , BSN preferred or comparable experience Case Management Certification preferred; Basic Life Support Certification required Minimum of 5 (five) years of clinical experience in acute care facility, case management preferred. Excellent communication and customer relation skills required.
