The Wound Ostomy Care Nurse develops, recommends, and implements policies, tools, guidelines, procedures, products and equipment for prevention of skin breakdown and management of skin/wound breakdown. This individual assesses, plans, implements and evaluates the plan of care for patients with complex management of wounds, fistulas and ostomies. Provides care to patients of adolescent age through older adult. Identifies patient's who are at high risk for developing ulcer or skin injury. Collaborates with staff in treatment and management of existing ulcer/skin breakdown.
